Quality and performance improvement in primary care can be a challenging endeavor. Without a plan, buy-in from stakeholders, accountability, and the
right tools and tactics, efforts to improve care delivery and outcomes will likely
have suboptimal results at best. This presentation will discuss an approach
that proved measurably successful, resulting in local, state, and national
recognition. By diving in from planning down to tools and tactics, participants
will leave with ideas to incorporate into their own approach to quality and
performance improvement.
